Chelsea have officially named their starting line-up and substitutes to play host to already relegated Sunderland in their final game of the Premier League season.

The Blues will pick up the league title after the match and will be keen to do so on a high after seeing off David Moyes' men in front of the home crowd at Stamford Bridge.




Looking to get the job done, Blues manager Antonio Conte has Thibaut Courtois between the sticks, while at the back he goes with a three of Cesar Azpilicueta, David Luiz and John Terry. Cesc Fabregas and N'Golo Kante are both picked in midfield.

Further up the pitch, Chelsea name Willian, Diego Costa and Eden Hazard to carry the attacking threat.

If Conte needs to change things at any point during the 90 minutes he can turn to his bench, where Michy Batshuayi and Pedro Rodriguez are available.
