Kick-off: 15:00 (UK time)

West Ham United have officially announced their team and substitutes to take on Stoke City at the Britannia Stadium on the Premier League's final matchday.

The Hammers are aiming to overhaul Manchester United to finish in fifth spot in the table and head into the game against the Potters sitting sixth and just a single point behind Louis van Gaal's men, who host Bournemouth.

Hammers boss Slaven Bilic, whose men head into the game on the back of an epic 3-2 win over Manchester United  on Tuesday night, has Andy Carroll leading the line, while Diafra Sakho also plays. Manuel Lanzini will look to create the chances for the pair, while Michail Antonio also features. Mark Noble captains the side from midfield.

Winston Reid and Angelo Ogbonna are the central defensive pairing, while on the bench Bilic can turn to attacking pair Enner Valencia and Emmanuel Emenike if needed.
