Chelsea have named their starting eleven and substitutes to play host to David Moyes' West Ham United side at Stamford Bridge this afternoon.
The Blues are facing an uphill battle to reach the top four in the Premier League and sit a whopping eleven points behind fourth placed Tottenham Hotspur, meaning dropping points is unthinkable if they are to keep alive faint hopes of Champions League football next season.

Blues boss Antonio Conte picks Thibaut Courtois in goal, while Gary Cahill, Cesar Azpilicueta and Antonio Rudiger are the back three. N'Golo Kante and Cesc Fabregas pair up in midfield, while Willian, Alvaro Morata and Eden Hazard are the attacking threats.
